Primary Health Care, Inc. (PHC) has provided health care services at CISS since 1988 and is proposing to add a nurse care management program to improve the quality of care delivered to this vulnerable population. The goals of the PHC/CISS Nurse Care Manager Program are to:
- To increase access to coordinated, health care services for the homeless by providing triage and assessment for an additional 8 hours per day (increasing medical presence from 28 hours/week to 68 hours/week).
- To better manage chronic health care needs of high utilizers of care.
- To increase availability of prevention and wellness services for this population. The Nurse Care Manager will achieve results by connecting clients with a primary care provider, increasing the completion of wellness exams and health risk assessments, reducing ER utilization rates and EMS transports, providing care management and education to high utilizers, and increasing client participation in wellness programs at CISS.
