The Polk County Continuum of Care (CoC) is an independent nonprofit organization that facilitates our community's response to homelessness. The Des Moines-based organization focuses on collaboration and setting the homelessness response strategy for the community. We distribute $3.6 million in funding from the U.S. Department of Housing and Urban Development (HUD) and others across the county. Our work is to coordinate resources and services, identify needs, and disseminate best practices related to homelessness. The Polk County Continuum of Care serves1,500 Polk County residents experiencing homelessness each year by building a comprehensive network of support in our community. We partner with over 100 organizations in our community including homeless service providers, local and state agencies, foundations, mental health centers, individuals experiencing homelessness, and the business community grounded in our foundational beliefs of Racial Equity, Community Collaboration, and Housing First.
