In 2019, Des Moines/Polk County was awarded a transformative grant by the U.S. Department of Housing and Urban Development to address youth homelessness. As one of 23 communities nationwide selected for the Youth Homelessness Demonstration Program (YHDP), Polk County has a unique opportunity to increase resources to prevent and end youth homelessness. Mid-Iowa Health Foundation provided an initial planning grant that led to Polk County being accepted into this program and continues to invest in the work.
